Q: How can I change my policy beneficiary and/or contingent beneficiary? A: As the owner of a policy, you may change your primary or contingent beneficiary at any time provided the beneficiary designation is revocable. To change your primary or contingent beneficiary, please request the Change of Beneficiary Form (Form 3116-NSLAC). Q: How do I report a claim? A: You may call toll-free, 877.446.6010 (press 2 for claims); fax 513.794.4521; or e-mail our Claims Department at NSClaims@nslac.com. Q: What information do I need to provide for a claim? A: For a death claim: We will require basic information regarding the deceased: name, address, policy number and any other identifying information such as Social Security number, date of birth, and cause of death, if known. For each beneficiary, we will require: name, current address and phone number and Social Security number. See "What do I need to do to get a death claim processed?" below for additional requirements. For a Crediting of Specified Amount for Total Disability claim: We will require the name of the claimant; current address; phone number; Social Security number; date of commencement of disability; and any information regarding the cause of disability, length of disability (if known), etc. Please request the Disability Form (Form 9194-NSLAC). Q: Does National Security offer automatic checking withdrawal for monthly premium payments? A: Yes, we offer automatic bank checking (ABC) withdrawal on a monthly basis.
